Broker Package
Thank you for your interest in the Mortgage Financial Services Wholesale Mortgage Broker Program. Please submit all of the following information for our review.
ALL OF THESE ITEMS ARE NEEDED FOR FULL APPROVAL.- Executed Broker Agreement
- Completed Lending Originator Application
- Executed IRS form W-9 including Federal Taxpayer ID Number (TIN). The payee name associated with the TIN must be consistent with the name and TIN used on tax returns.
- Copy of your current State License, Registration or Charter (as applicable) stating company name including DBA's, and business address.
- Resumes of Principal Officers of the company ( including employment history).
- FHA / VA Agreement (For VA approval include a check for $100.00 payable to Veteran's Administration.)
